Gala GLP-1 Telehealth Informed Consent
I. Introduction
This Telehealth Informed Consent (“Consent”) is made between you (“Patient,” “you,” or “your”) and AI Coaching Inc., d/b/a Gala GLP-1 (“Gala GLP-1,” “we,” or “us”). By using our telehealth platform, you acknowledge that you have read, understood, and agree to the terms outlined in this Consent, in accordance with applicable state and federal laws.
II. Description of Telehealth Services
Telehealth involves the use of electronic communications (e.g., video, phone, messaging) to enable healthcare providers at different locations to evaluate, diagnose, and/or treat patients. Through Gala GLP-1, you are connected with licensed healthcare providers who may recommend a course of treatment, which could include prescription medication if deemed medically appropriate. Prescriptions, if any, are dispensed by our partner pharmacy, not by Gala GLP-1.
III. Nature, Benefits, and Limitations
- No Physical Examination: Telehealth does not replace a full medical evaluation that might otherwise occur in an in-person setting.
- Potential Risks: These include possible delays in medical evaluation due to technology failures, information security risks, or misunderstandings arising from the lack of in-person interaction.
- Not for Emergencies: If you have a medical emergency, call 911 or go to the nearest emergency room immediately. Our telehealth services are not intended for urgent or life-threatening conditions.
- No Guarantee of Prescription: Any prescription is provided solely at the discretion of a licensed provider, based on your individual medical history, symptoms, and best clinical judgment.
- Results May Vary: Individual responses to any recommended treatment can differ. Testimonials or success stories are not guarantees of specific outcomes.
IV. Patient-Provider Relationship with Gala GLP-1
- Licensed Providers: Gala GLP-1 collaborates with a network of licensed healthcare providers. Each provider is responsible for their own professional judgment, advice, and treatment decisions.
- Scope of Service: Gala GLP-1 itself is not a medical group or pharmacy. We facilitate telehealth consultations but do not directly dispense medication.
- Service Availability: Our platform strives to serve patients across multiple states. However, provider availability and telehealth regulations may vary by location.
V. Consent to Use Telehealth and Data Transmission
- Information Sharing: By using this service, you consent to the transmission of your personal health information via secure telehealth technologies. Your information may be shared with partner pharmacies and other necessary third parties to coordinate care.
- Privacy and Security: Gala GLP-1 complies with HIPAA (or equivalent privacy regulations) to protect the confidentiality of your health information. Despite best efforts, data breaches or unauthorized access may still pose a risk.
VI. Voluntary Participation and Right to Withdraw
- Voluntary Use: Your use of Gala GLP-1’s telehealth platform is voluntary. You may choose to stop or refuse treatment at any time without affecting your right to future care or treatment.
- Right to Seek In-Person Care: You have the right to pursue in-person medical services at any time if you prefer or if the telehealth provider determines it is necessary.

- No Warranties: Gala GLP-1 makes no express or implied warranties about the accuracy, reliability, or completeness of the telehealth services provided through our platform.
- Limitation of Liability: By proceeding with telehealth services, you agree not to hold Gala GLP-1 liable for any harm or injury resulting from the use of telehealth, except as required by law.
- Acknowledgment: You acknowledge that you have had the opportunity to ask questions about the information in this Consent and that all questions have been answered to your satisfaction.
